SDF and the perfect tenses

The perfect tenses in an anamnesis
  1. Present Perfect: We use the present perfect to describe actions that occurred at an unspecified time before now. We are interested more in the result of that past or continuing action now.
    1. Interrogative: "Have you experienced any pain in your chest?"
    2. Negative: "I have not had any allergic reactions previously."
    3. Affirmative: "The patient has reported feeling dizzy previously."
  2. Past Perfect: We use the past perfect to indicate that an action was completed before another action in the past.
    1. Interrogative: "Had you taken your medication before your symptoms worsened?
    2. Negative: "The doctor said she had not seen such a case before.
    3. Affirmative: "He had experienced similar symptoms before visiting the clinic."
  3. Future Perfect: We use the future perfect to describe actions that will be completed before a specified point in the future.
    1. Interrogative: "Will you have completed your treatment by the time we schedule your follow-up appointment?
    2. Negative: "I will not have finished my treatment before the next appointment."
    3. Affirmative: "The patient will have recovered fully by the end of the month.

Patient Consultation

OSTEOPATH: Hello, how may I help you today? I have been looking forward to understanding your concerns better.

PATIENT: Hi, I've been experiencing some persistent back pain for the past few weeks. It has been bothering me, especially during my daily activities.

OSTEOPATH: I see. Since when have you been feeling this discomfort? I want to ensure that we address this properly.

PATIENT: It started about three weeks ago. I thought it would go away on its own, but it has only gotten worse.

OSTEOPATH: That sounds challenging. During this time, have you noticed any specific activities that aggravate the pain?

PATIENT: Yes, sitting for long periods has been particularly uncomfortable. I have also felt some stiffness in the mornings.

OSTEOPATH: Thank you for sharing that. Have you had similar pain in the past, or is this the first time you have experienced it?

PATIENT: I have had mild discomfort before, but it has never lasted for this long. Since the pain started, I have been struggling to sleep well, and I have felt quite fatigued during the day.

OSTEOPATH: That must be frustrating. Have you been doing anything to manage the pain during these past three weeks?

PATIENT: Yes, I have been trying to stretch and use a heating pad. I have also taken some over-the-counter pain relievers, but they haven’t been very effective.

OSTEOPATH: I understand. Have you had any injuries in the past that could be related to your current discomfort?

PATIENT: Not that I can think of. However, I have been working from home for the past year, and I have been sitting for longer hours than usual.

OSTEOPATH: That could be a contributing factor. Since you started working from home, have you changed your workstation setup?

PATIENT: Not really. I have been using my dining table as a desk, and I don’t think my chair provides much support.

OSTEOPATH: That makes sense. Poor posture during extended sitting periods can lead to muscle strain. Have you noticed any pain radiating to other areas, like your legs or shoulders?

PATIENT: Yes, I have felt some tension in my shoulders and neck, especially during the afternoons. I have also had occasional tingling sensations in my lower back.

OSTEOPATH: That is important to note. I have been treating similar cases, and I believe we can work together to find a solution. Before we begin, I will conduct a thorough examination to assess your condition. Please undress down to your undergarments so I may better examine you.
